Main ConfidentialsponsibilitiesWe have part time opportunities in our South Ribble team. You will work as part of the Health Visiting team, working closely with Health Visitors and specialist safeguarding nurses.
Working within Lancashire Child and Family Wellbeing Services offers a unique environment, with clear visibility of senior management who are easily approachable due to their hands-on approach, which includes our Managing Director. You will feel listened to and be given the platforms to have your voice and ideas heard.
Confidentialsponsibilities include but are not limited to:
Deliver the healthy child programme at four levels as delegated by the Specialist Community Public Health Practitioner SCPHN
Ability to Confidentialcognise health needs for individual clients and situations in Confidentiallation to the health of children, young people,individuals and families, sometimes in complex situations, identifying situations where Confidentialferral for support from SCPHN is necessary
Prepare for, attend and participate in Confidentiallevant inter-agency meetings as delegated by the SCPHN and undertake health needs assessments and care planning as appropriate
Support the SCPHN in providing specialist targeted support to meet the needs of children identified as at risk or with complex needs and ensure Partnership Plus Plan in place
As appropriate, host virtual groups and sessions on public health topics to support families and young people
As this is a community role, you will also have a valid UK driving licence, with access to a car for work purposes. Although we can make certain accommodations if you do not yet have a driving licence or are unable to hold one.
